PLYO Physical Therapy LLC, operating publicly as PLYO Physical Therapy (“PLYO,” “we,” “us,” or “our”), respects the privacy of people who visit plyopt.com. This Website Privacy Policy explains the types of information that may be collected through our website, how that information may be used or shared, and how to contact us about privacy questions or requests.

Important: This Website Privacy Policy is different from PLYO’s HIPAA Notice of Privacy Practices. The HIPAA Notice explains PLYO’s duties and your rights regarding protected health information when HIPAA applies.

1. Information We Collect

Information You Provide

When you use our Request Appointment form or otherwise contact us through the website, you may provide information such as:

  • Your name, phone number, email address, preferred contact method, and preferred contact time;
  • The reason you are contacting PLYO or the care path you are interested in;
  • Health-related information you choose to provide, such as the body area or condition involved, how long the issue has been present, surgery information, prior physical therapy, and your goals;
  • A broad insurance or payment category; and
  • Optional information you enter in a free-text field.

The Request Appointment form does not require you to sign in to a Google account. Your email address is collected only if you enter it in the form.

Information Collected Automatically

Like most websites, ordinary browser, hosting, and server infrastructure may generate technical information when you visit or use the site. Depending on the technology involved, this may include an Internet Protocol (IP) address, browser or device information, request times, pages or resources requested, and similar technical data. Google-hosted content used by the embedded appointment-request form may also use cookies or similar technologies.

Information We Do Not Request Through the Appointment Form

The current Request Appointment form does not ask you to provide:

  • A Social Security number;
  • Payment-card information; or
  • File uploads.

Please do not submit these items or other sensitive information that is not requested or needed for your inquiry.

2. How We Use Information

We may use information collected through the website to:

  • Respond to appointment requests and other inquiries;
  • Contact prospective or current patients using the contact information they provide;
  • Understand the reason for an inquiry and help identify an appropriate starting point;
  • Respond to benefits, insurance, or payment-related questions when requested;
  • Operate, maintain, secure, and troubleshoot the website and related technology;
  • Meet legal, regulatory, recordkeeping, and compliance obligations; and
  • Protect PLYO, our patients, website users, or others when permitted or required by law.

We do not use the current website form for behavioral advertising or targeted advertising.

3. Google Forms and Google Workspace

PLYO’s Request Appointment form is embedded on plyopt.com from Google. The form itself is hosted and processed by Google rather than by WordPress. When you submit the form, the response is stored in PLYO’s Google Forms / Google Workspace environment, where an authorized PLYO user can review it.

Under the current configuration, WordPress does not receive or store the substantive fields that you submit through the Google Form, and the form responses are not copied by WordPress into a CRM, webhook, automation, spreadsheet, or WordPress database.

PLYO receives a generic notification that a new response has been submitted; the full response is reviewed within Google Forms rather than included in that notification email.

4. Cookies and Similar Technologies

The website and Google-hosted resources used by the embedded form may use cookies or similar technologies in connection with operating and providing those website features. The specific technologies used by third-party services may change over time.

Based on the current live-site configuration, PLYO does not use Google Analytics/GA4, Google Tag Manager, Google Ads tracking, Meta Pixel, Hotjar, Microsoft Clarity, session-recording tools, heatmaps, or similar analytics or advertising tracking tools on plyopt.com.

If PLYO materially changes its use of analytics, advertising, or similar tracking technologies, we will update this policy as appropriate.

5. How Information May Be Shared

PLYO may disclose or permit access to information in limited circumstances, including:

  • To service providers that help us operate the website or provide technology services, such as Google for Google Forms/Workspace-related services and our website-hosting provider;
  • To professional advisers, consultants, or service providers when reasonably necessary for legitimate business, compliance, or legal purposes;
  • To government, regulatory, law-enforcement, or other authorities when disclosure is required or permitted by applicable law; and
  • When reasonably necessary to protect the rights, safety, security, or property of PLYO, our patients, website users, or others, as permitted by law.

PLYO does not currently sell personal information for monetary consideration.

6. HIPAA and Health Information

Some information you submit through the website may relate to your health, health care, or payment for health care. Not every visit to our website or every piece of website data is automatically protected health information (PHI) under HIPAA.

When information you submit through the website constitutes protected health information under HIPAA, PLYO’s HIPAA Notice of Privacy Practices also applies to that information. The Notice of Privacy Practices explains how PLYO may use and disclose PHI, your HIPAA rights, and how to contact PLYO about those rights.

If there is a difference between this Website Privacy Policy and the HIPAA Notice of Privacy Practices regarding PHI, the HIPAA Notice and applicable law govern the PHI.

7. Data Retention

Retention periods may vary depending on the type of information and why it was collected. PLYO may retain website-inquiry information for as long as reasonably necessary to respond to an inquiry, manage appropriate follow-up, meet legal or recordkeeping obligations, resolve disputes, protect legal rights, or support legitimate operational needs.

If information submitted through the website becomes part of a patient or clinical record, different health-record retention requirements may apply. PLYO does not state a fixed retention period for all website inquiries because the appropriate period can depend on the nature and use of the information.

8. Data Security

PLYO uses reasonable administrative, technical, and physical safeguards appropriate to the nature of the information we maintain. These measures are intended to reduce the risk of unauthorized access, use, disclosure, alteration, or loss.

No website, Internet transmission, or electronic storage method can be guaranteed to be completely secure. Please use care when submitting information online and provide only information reasonably necessary for your request.

9. Children and Minors

This website is intended for a general audience and is not directed to children under 13. If information is being submitted for a child under 13, a parent or legal guardian should submit the appointment or contact information on the child’s behalf.

PLYO may provide health care services to minors when appropriate; this section concerns use of the website and does not create an age restriction on receiving physical therapy services.

10. Third-Party Links and Services

Our website may include links to or content provided by third-party websites or services. PLYO does not control the privacy practices of independent third parties. When you leave plyopt.com or interact directly with a third-party service, that third party’s privacy terms may apply.

11. Your Privacy Choices and Requests

You may contact PLYO if you have questions about this Website Privacy Policy, want to correct information you submitted, have a privacy concern, or want to make a request regarding personal information.

Depending on the information involved and the law that applies, you may have specific rights regarding your personal information. We may need to verify your identity or authority before acting on certain requests.

If your request concerns protected health information subject to HIPAA, the rights and procedures described in PLYO’s HIPAA Notice of Privacy Practices apply.

12. Changes to This Privacy Policy

We may update this Website Privacy Policy when our website practices, technology, services, or legal requirements change. When we make a material update, we will post the revised policy on plyopt.com and update the effective date.
