Which Starting Point Fits You Best?

Not every patient needs the same starting point. Choose the option that best fits your situation. If you are not sure, select Help Me Choose.

Start General PT

  • New injuries or recent flare-ups
  • Post-operative rehab
  • First-time physical therapy
  • Medicare-covered care
  • More straightforward orthopedic problems

Request Complex PT Evaluation

  • Symptoms that have not resolved with prior care
  • Prior PT helped temporarily but did not last
  • Conflicting advice or unclear answers
  • Imaging, injections, medications, surgery, or other PT without full resolution
  • Seeking a conservative option before more medical intervention

Help Me Choose

  • You are unsure which path fits
  • Questions about benefits, cost, or scheduling
  • Help choosing the right starting point

You can still use the form or contact us directly. Let us know that your main question is about benefits, payment, or cost, and we will do our best to discuss those details before care begins.

Please bring a photo ID, your insurance card if applicable, any relevant imaging reports or referral information, and comfortable clothing that allows you to move. If you have had prior treatment, it can also help to bring notes about what has or has not helped.

When appropriate, yes. Care at PLYO may include hands-on treatment, movement testing, exercise guidance, education, and a home strategy. The exact plan depends on your goals, exam findings, and how your body responds.

If physical therapy does not appear to be the right next step, we will discuss that honestly. The goal is to help guide you toward the most appropriate path, which may include medical follow-up, imaging, referral, self-management, or another option.

We do our best to respond as soon as possible during normal business hours. Response time may vary depending on the day, but we will follow up with next steps, scheduling options, or questions needed to better understand your situation.
