Discrimination Is Against the Law

PLYO Physical Therapy LLC, operating publicly as PLYO Physical Therapy (“PLYO”), complies with applicable Federal civil rights laws and does not discriminate on the basis of race, color, national origin (including limited English proficiency and primary language), sex, age, or disability. PLYO does not exclude people or treat them less favorably on any of these bases.

PLYO provides the following services when required to ensure equal access to its health programs and activities:

• Reasonable modifications for individuals with disabilities and appropriate auxiliary aids and services, free of charge and in a timely manner, when necessary. These may include qualified interpreters, including sign-language interpreters, and information in alternate formats such as Braille, large print, audio, or accessible electronic formats.
• Language assistance services, including electronic and written translated documents and qualified oral interpretation, free of charge and in a timely manner, when such services are a reasonable step to provide meaningful access to an individual with limited English proficiency.

There is no charge to you for language assistance or auxiliary aids and services that PLYO is required to provide.

Civil Rights Complaints

If you believe that PLYO has discriminated against you or failed to provide required language assistance, reasonable modifications, or appropriate auxiliary aids and services, you may contact PLYO using the information above.

You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights (OCR).
